Referral Process for Residential Services

Home and Community Care (South West Local Health Integration Network - SW LHIN) administers the Hospice referral process, determining which clients are eligible and take priority for Hospice stays. Admission is based on the eligibility and the ability of St. Joseph's Hospice to accommodate palliative residents.

Referral Process for Community Services

Hospice accepts referrals from health care providers, self-referrals, and referrals from families and caregivers for individuals diagnosed with a palliative illness, their family or caregivers, or those who have lost a loved one in the last year may benefit from hospice support. If you would like to refer someone to community services, please find our referral form here.
