Information for Health Care Providers
Referral Process for Hospice Services (excluding Residential Services)
Hospice accepts referrals from health care providers, self referrals and referrals from families and caregivers. Individuals diagnosed with a palliative illness, their family or caregivers, or those who have lost a loved one in the last year may benefit from hospice support. If you would like to refer someone for community hospice services, please find our referral form here.
Referral Process for Residential Services
The South West Community Care Access Centre (CCAC) administers the Hospice referral process, determining which clients are eligible and take priority for Hospice stays. Admission is based on the eligibility and the ability of St. Joseph's Hospice to accommodate palliative residents. Information about our residence can be found here. Please consult the Southwest CCAC website for more information.

Partnering with the Health Care Community
Forming strong relationships with health care partners is a priority for Hospice to ensure that individuals have access to the highest quality of care. You and your colleagues are welcome to call Hospice to book a tour to see first hand how hospice services can enhance the quality of life for individuals with a palliative illness, caregivers and the bereaved. .
